Western Montana Mental Health Center Employee Benefit Summary Effective 1/1/2013
**Most benefits apply to employees who work 20 hours or more per week. * Retirement Plan * Employer provides 100% match to employee contribution after 1st year of employment. * 100% vesting of employer and employee dollars from day one. * American Fund Mutual Funds & Calvert (403b and/or ROTH Option) * 6 Year Graduated Match Schedule (0%; 1%; 2%; 3%; 4%; 5%) * Vacation – available after 6 mo, 88 hours/yr (levels incr @ 3/7/13/20 yrs) cap of 240 hrs * Holiday - paid leave 10 days/80 hours per year * Sick Leave - Illness 4 hours/month 1st yr; 8 hours /month 2nd yr and thereafter * Direct Grants of Sick Leave * Family Medical Leave – Per Federal Law * Professional Training Conferences - up to 40 hours/$500 annually * Employee Evaluations - Annually, or as scheduled * Clear Performance Expectations – As outlined per the Job Description * Grievance Rights - as outlined in the Personnel Manual * Relocation Expenses - Negotiated at time of hire * 90 Day Waiting Period * Health Insurance * 1st dollar coverage for professional services (in network) * 70/30% co-pay for office visits & non deductible medical costs * $500 deductible for major medical (primarily hospital outpatient/inpatient) * 70/30% co-pay on major medical up to $4,500 out-of-pocket for single; $9,000 for family * Use of Health Link Hospitals Network for discounts; 25% penalty for nonHLHN * 1st dollar coverage for preventive services - services reimbursed @100% of allowable fees * $300 deduction for prescription drugs, then $10/25/50 co-pay, deductible waived for generic * HRA Employer Contribution * 80/20% co-pay after deductible is met * $2,500 deductible single, $5,000 family, pay for all office visits * 80/20% co-pay on major medical up to $4,000 out-of-pocked for single; $8,000 for family * Use of Health Link Hospitals Network for discounts; 25% penalty for non-HLHN * 1st dollar coverage for preventive services-services reimbursed @100% of allowable fees * HSA Employer Contribution: $100/monthy * Flex Plans * Long Term Disability - 60% of Monthly Earnings ($6,000 max) @Employee Option * Dental - 3 Plans: Low, Mid, High (with ortho) @ Employee Option (through Guardian) * Vision - VSP network @Employee Option (through Guardian) * Supplemental Health - AFLAC (Pretax/Taxed) @ Employee Option * Life * Voluntary/Supplemental Life - Additional coverage for employee, spouse, children
