Description of Services Provided:
Serves emotionally disturbed children, adolescents and their families with the ultimate goal of family preservation. The case manager assesses the child and family, develops a care plan in cooperation with the family and all concerned professionals, and coordinates communication and services.  Facilitating the provision of services needed by the youth and family, support for the family in times of crisis and conflict, and monitoring the child's progress at school and in the community enables the case manager to access and adapt the care plan as appropriate.  Case managers act as advocates to help families obtain necessary services and support.
