CSCT clients receive an intensive level of care which includes a comprehensive treatment plan designed to meet their mental health needs and support improved functioning within the school, family and community. Home Support Services An intensive in-home, family-focused program for children and adolescents who experience serious social, emotional and behavioral challenges. The overall goal of this program is to help parents and children learn skills to effectively manage and reduce challenging behaviors so that the child can function more effectively in the home, school and community. A Family Support Specialist consults with parents, children, and other professionals to create a customized treatment plan to fit the needs of the family, helping them learn important skills and concepts at their own pace and in their own home. Individual and Family Counseling Licensed professional counselors/social workers provide psychotherapy for children, adolescents and their families. Counseling is provided within a family systems context with specialized treatment for seriously emotionally disturbed children and adolescents. Outpatient psychotherapy may include individual and/or group therapy operating within the continuum of care offered by Western Montana Mental Health Center and following best practice models for short term, solution focused psychotherapy based upon clear and identifiable clinical needs. Psychiatric/Medication Services Managing emotional and behavioral challenges is a team effort and appropriate use of medication may be an important component. Our psychiatric staff, who are made up of both (Advanced Practice Registered Nurses (APRN’s) and psychiatrists, are able to assess, prescribe, and monitor clients who may need medication, and they provide education to both the client and their family members. Incident Notice Western Montana Mental Health Center Provides Notice of Data Security Incident MONTANA – July 17, 2025 – Western Montana Mental Health Center (“WMMHC”) is providing notice of a recent data security incident that may have involved personal and/or protected health information. WMMHC takes the privacy and security of all information within its possession very seriously. WMMHC has sent notice of this incident to potentially affected individuals and provided resources to assist them. On September 15, 2024, WMMHC discovered that it was experiencing a network disruption and immediately initiated an investigation of the matter with the assistance of independent cybersecurity experts. As a result of the investigation, it determined that certain files were accessed without authorization. WMMHC then engaged a third- party data review team to conduct a comprehensive review of those files, and, on or about May 27, 2025, learned that personal and protected health information belonging to WMMHC was contained within the potentially affected data. Based on WMMHC’s review of the potentially affected data, the following information may have been involved in the incident: Social Security numbers, driver’s license numbers, dates of birth, state or federal identification numbers, medical information, financial account information, and/or health insurance information. Of note, not all information was involved for all individuals, and WMMHC has no evidence of the misuse, or attempted misuse, of any potentially affected data. As soon as WMMHC discovered this incident, it took the steps described above and implemented measures to further enhance the security of its network environment and minimize the risk of a similar incident occurring in the future. WMMHC also notified the Federal Bureau of Investigation and will cooperate with any resulting investigation. WMMHC has established a toll-free call center to answer questions about the incident and address related concerns. Call center representatives are available Monday through Friday from 9:00 am to 9:00 pm Eastern Time, excluding holidays, and can be reached at 1-855-202-1403. The privacy and protection of personal and protected health information is a top priority for WMMHC. WMMHC deeply regrets any inconvenience or concern this incident may cause. WMMHC is providing the following information to help those wanting to know more about steps they can take to protect themselves and their information: What steps can I take to protect my personal information? Review Your Account Statements and Notify Law Enforcement of Suspicious Activity: As a precautionary measure, we recommend that you remain vigilant by reviewing your account statements and credit reports closely. If you detect any suspicious activity on an account, you should promptly notify the financial institution or company with which the account is maintained. You also should promptly report any fraudulent activity or any suspected incidence of identity theft to proper law enforcement authorities, your state attorney general, and/or the Federal Trade Commission (FTC). Copy of Credit Report: You may obtain a free copy of your credit report from each of the three major credit reporting agencies once every 12 months by visiting http://www.annualcreditreport.com /, calling toll-free 1-877-322-8228 , or by completing an Annual Credit Report Request Form and mailing it to Annual Credit Report Request Service, P.O. Fraud Alert: You may want to consider placing a fraud alert on your credit report. An initial fraud alert is free and will stay on your credit file for at least one year. The alert informs creditors of possible fraudulent activity within your report and requests that the creditor contact you prior to establishing any accounts in your name. To place a fraud alert on your credit report, contact any of the three credit reporting agencies identified above. Additional information is available at http://www.annualcreditreport.com. Security Freeze: You have the right to put a security freeze on your credit file at no cost. This will prevent new credit from being opened in your name without the use of a PIN number that is issued to you when you initiate the freeze. A security freeze is designed to prevent potential creditors from accessing your credit report without your consent. As a result, using a security freeze may interfere with or delay your ability to obtain credit. You must separately place a security freeze on your credit file with each credit reporting agency. In order to place a security freeze, you may be required to provide the consumer reporting agency with information that identifies you including your full name, Social Security number, date of birth, current and previous addresses, a copy of your state-issued identification card, and a recent utility bill, bank statement or insurance statement. Adult Treatment Services Adult Day Treatment The Day Treatment program provides education and support to clients who have been referred by a member of their treatment team. Day treatment gives people access to a variety of group and individual activities in a positive environment that encourages healthy social skills. Our goal is to provide education and support that will help people manage their mental health symptoms and gain skills and knowledge to achieve or maintain independent living. Adult Group Homes Adult group homes combine housing and services in a structured and supportive setting. Our group homes provide 24-hour support to our residents. Western Montana Mental Health Center staff are available to assist residents in building independent living skills and help them reach recovery to live successfully in the community or in Western client housing. Clinical staff is available to assist residents with reaching their recovery goals and working with other members of the treatment team to ensure success. Clients are referred to group home services by members of the treatment team or other agencies. Crisis Stabilization Facility Crisis Stabilization facilities provide short term, intensive, inpatient support for adults experiencing a life-threatening crisis. Most clients stay at the crisis facility for 3 to 7 days. During their stay clients work with their treatment team and crisis facility staff to resolve the crisis and to develop a plan for the client’s care after leaving the crisis facility. Client Housing Western Montana Mental Health Center offers a variety of housing options with Western-owned apartments and/or homes and acts as landlord to provide independent living arrangements. Residents may be supported by a case manager and community-based rehabilitation services to assist them with day-to-day living skills. Client housing helps integrate clients into the community while maintaining supportive services that meet residents’ needs as they become more independent. Jail Diversion Jail Diversion Programs provide support and treatment for individuals experiencing mental illness during pre-arrest, incarceration, and after release from county detention centers. The Jail Diversion team works collaboratively with detention center officers, local law enforcement, and other components of the criminal justice system to provide services within the county detention center and to assist in diversion from incarceration. Services provided while incarcerated can include mental health assessments, case management, therapy, peer-support services, group therapies, crisis intervention, and chemical dependency evaluations. These services reduce recidivism by connecting individuals to behavioral health services. Outpatient Therapy Outpatient therapy is offered across the life span and may include individual and/or group therapy designed to help clients address symptoms that interfere with functioning in one or more life domains such as familial, social, educational or occupational. Western Montana Mental Health Center believes that the client’s voice is central to the success of its outpatient therapy programs. The Partners for Change Outcomes Management System is an evidence-based tool that helps us work with clients to set goals, ensure an effective therapeutic relationship, and to achieve the outcomes identified by the client. Program of Assertive Community Treatment (PACT) Program of Assertive Community Treatment (PACT) is designed to help people with serious mental health conditions live independently in the community by providing comprehensive mental health and supportive services. Clients in PACT receive a combination of services depending on each client’s individual needs. These services include medication management, nursing support, case management, therapy, substance abuse treatment, employment and housing services, crisis intervention, and peer support. This program is for clients who need more help than is available through regular outpatient services and is offered 24 hours a day, 365 days a year. Psychiatric/Medication Management Services Managing mental illness is a team effort and appropriate use of medication is an important component. Our providers, who are made up of Advanced Practice Registered Nurses (APRNs), Physicians Assistants, and psychiatrists, with the help of their trained nursing staff, are able to assess, prescribe, and monitor clients who need medication to treat their illness while providing education to both the client and their loved ones. Vocational Services Vocational services provide adults living with severe mental illness support to attain their employment goals. Utilizing evidence-based practices, supports are based on choice and focused on assisting individuals in finding and keeping jobs that capitalize on their personal strengths and motivation. Vocational services are recovery-driven, emphasizing skill development through a multidisciplinary team approach. Job-hunting, on-the-job, and post-employment assistance is offered from resume and cover letter writing, interview preparation, workplace conflict resolution, accommodation planning, work-related symptom management, and benefits management.
